We started because the sickest patients were the hardest to reach.
The patients who need a doctor most are often the ones who can least get to one. That is the whole problem, and American primary care is not built for it.

The story
A fifteen-minute office slot assumes you can get to the office.
So we started making house calls. One bag, one car, a list of patients who had stopped showing up to their appointments.
What became obvious quickly: seeing someone at home tells you things a clinic never will. The expired insulin in the door of the fridge. The throw rug at the top of the stairs. The daughter who has been managing all of it alone and has not slept properly in a year.
Today we are a team of clinicians across Chicagoland. The model has not changed.
